EXCHANGING CARE MODELS BETWEEN SINGAPORE & BARCELONA
Healthcare and Social Care usually evolve as two separate systems; but by breaking the siloed institutions and creating Ecosystems of Care we can achieve a more holistic support for vulnerable people and effective synergies amongst Care professionals.
In May 2019, The Care Lab organised and hosted part of a study trip from the Ministry of Health Transformation Office in Singapore who were coming to Barcelona to learn about best practices in Integrated Care Models and Community-Based Care services. Visiting the unique local programs of Radars, Vincles and Social Superilles, and organising a lively debate with local service users, gave the government team valuable insights and firsthand experience of how such community-based systems of care are initiated, and built bottom-up by the community themselves.
