DEMENTIA CARE BEYOND WALLS
According to the WHO, Dementia is a major cause of disability and dependency among older adults worldwide. Studies show that being in a familiar environment and keeping meaningful relationships can mitigate the psychological distress and behavioural symptoms of people with dementia.
Care Beyond Walls is a new community-based model of care we co-designed in Singapore to support people with dementia and their caregivers. The model comprises four main elements: a redesigned dementia daycare centre, a new social space to encourage community engagement, a personalised care plan enabling people with dementia to contribute to their neighbourhoods using their skills, and community interventions to promote dementia-friendly principles, spaces, programming, and services.
