


Co-creating a plan allowing informal caregivers for self-care

EMPOWERING CAREGIVERS IN SELF-CARE
Informal caregivers often sacrifice their well-being to meet the needs of those they care for. Many are unaware of the impact of their role, making the journey to self-care even more challenging. Research indicates that long-term caregiving can deteriorate health over time.
In this project, we co-created a plan for home care teams to support and activate informal caregivers and enhance their quality of life. This plan identifies overloaded, vulnerable, or isolated ICs, raises awareness about their role and self-care needs, offers strategies for self-care through a personalised action plan, and establishes a monitoring and assessment framework for ongoing support.
