Integration is one of the biggest opportunities in the healthcare sector to improve care delivery and patient experience globally.
Since 2022, we have worked with Boehringer Ingelheim and the NHS to build on existing Heart failure and Chronic Kidney Disease pathways and create a blueprint to improve integration, outcomes, and experience in different regions.
Using a multi-stakeholder journey mapping methodology, each system transformation project aims to understand patients' key issues and needs and the diverse healthcare professionals involved in the pathway from across the system. It also aims to identify opportunities for integration, transformation, and improvement of patient care.
