Co-creating the new protocol with a multi-disciplinary care team 

NEW INTEGRATED HOME CARE MODEL

Often, older adults and socially isolated people undergoing hospital admissions lack the social and family support needed to be able to return home early and recover better, which results in a longer time in the hospital, delaying their recovery and saturating the system.

In this participatory project, we co-designed a new integrated care model that combines healthcare and social support with technological assistance and monitoring to shorten patients' hospital stays and provide holistic support in their return home and recovery.

As a result, a new home care service will be implemented consisting of daily home support by a professional caregiver, an Individualized Care Plan, a Monitoring Kit, and a coordination platform. 

Five co-creation workshops were conducted with patients, caregivers, professional caregivers, multidisciplinary health and social field teams, home service providers, technology experts, and decision-makers.
